8D Problem Solving

Who will benefit?

The workshop will benefit those responsible for ensuring quality including senior managers, team leaders, quality personnel, engineers, operators and anyone interested in understanding and applying robust problem solving techniques or those wishing to know more about quality.

Workshop Objectives

8D problem solving is a tried and tested method for ensuring issues that have impacted on the customer or within the business are addressed and prevented from occurring again in the future.

Originating in the automotive sector but now used in most sectors, the cross-functional team methodology can be successfully applied to both products and services and enables an organisation to:

  • Establish the facts to define the issue:

  • Protect the customer by implementing containment;

  • Consider potential causes of the issue;

  • Establish root causes of the issue from a product or service defect, lack of detection and systemic failure perspective;

  • Identify corrective actions that will eliminate the risk of recurrence in the future.

  • Apply ‘Lessons Learned’ principles to other products and/or services to prevent occurrence.

By applying, adopting and embracing the approach, an organisation will ensure it does its very best to reduce the number of customer and internal issues, improve customer satisfaction, satisfy specific requirements of quality management standards (including ISO9001:2015, ISO13485:2016 and ISO/TS16949:2009) and reduce avoidable and unexpected costs which will in turn enhance company profits.

Practical exercises are used throughout the workshop. Real company examples can be included, where appropriate.

The outline aims of the workshop are to enable the delegates to:

  • Understand the importance of a systematic approach to dealing with customer, supplier and internal issues;

• Understand the importance of a team approach and getting the right team members on board;

• Why avoiding jumping to conclusions is vital:

• Establish the facts through 5W/2H to define the issue;

• Establish how to contain the issue;

• Establish potential causes of the issue through team brainstorming;

• Narrow down potential causes through Go-Look-See and other exercises including data collection;

• Establish product/service, lack of detection and systemic failure causes through 3 x 5 Why;

• Identify appropriate actions to prevent recurrence of issue;

• Apply ‘Lessons Learned’ to its products and/or services where appropriate;

• Understand the approach needed at each stage by the person leading the activity to ensure it is a success and how to get management buy in;

• Understand equipment and facilities needed to support effective 8D problem solving.
